What to include

  • Practice name, logo and main line — not personal mobiles for staff.
  • Credentials in the title (DDS, RN, PA-C) so patients know who they've reached.
  • A 'Book an appointment' link to your scheduling system.
  • The confidentiality wording your practice approved, in the disclaimer slot.

What works for healthcare

  • 1

Use the confidentiality wording your practice approved; the sample line is a placeholder.

  • 2

Show the practice's main line, not personal mobiles, for front-desk and clinical staff.

  • 3

A portrait helps for providers; front-desk staff can use the compact version.

Common questions

Does a signature disclaimer make email HIPAA compliant?
No. A confidentiality notice is good practice but doesn't change how email is protected. It reserves space for the wording your compliance officer requires; secure messaging is a separate matter.
Can the whole practice use one signature?
Yes. Applied across a Google Workspace domain, one design gives every provider and staff member the same practice details, with their own name and credentials filled in.

Every template here is free to customize in the signature generator and install yourself. If your company runs on Google Workspace, the same design can be applied to every employee from the admin side — see how team management works.
